Skin Checks Voucher Terms & Conditions

  • By ordering this Skin Check Voucher (Voucher), you accept these terms and conditions, and those of your chosen participating MoleMap or MoleMap partner clinic (Clinic) who will conduct your Skin Check (Skin Check)
  • This Voucher is provided by Bodycare Health and Wellbeing Pty Ltd (ABN 19 624 842 351 trading as Healthworks® (Healthworks, we, us, our), but the Skin Check will be provided to the customer by the Clinic who is wholly responsible for the Skin Check.
  • Healthworks and its employees, directors and related bodies corporate will not be responsible for any injury, loss, or damage you or the customer may suffer arising out of or connected with the Skin Check.
  • Once ordered you will receive the Voucher by email. Each voucher can be redeemed for one Skin Check. The Voucher will be addressed to the company and is not transferable. Any unused Vouchers cannot be redeemed for cash or credit.
  • The customer must apply their Voucher when booking their appointment.  Customers who book their Skin Check without a Voucher may be charged the Clinic’s standard rate for this service.
  • Once a customer uses a Voucher in the booking process, it is deemed to have been used.
  • If the selected service’s value exceeds the value of the Voucher, the remaining balance must be paid by an alternative form of payment.
  • No cashback will be provided if the cost of the purchased service is less than the value of the Voucher.
  • The Voucher will contain a unique code which must be provided when the customer is booking online. The Skin Check is valid until the date advised on the Voucher, subject to availability. Please note dates/times at each clinic are limited. We encourage customers to redeem their Voucher as early as possible to avoid missing out.
  • Customers should arrive 5 minutes prior to their booked appointment. During the appointment, they will be asked to complete a consent form and may be asked questions about their medical history, existing health conditions and history of sun exposure. If they do not sign the consent form and provide the requested information the Clinic will not be able to conduct their Skin Check.
  • Use of a Voucher is subject to MoleMap’s cancellation and no-show policy.

PRIVACY STATEMENT

  • We will collect your personal information provided including your name and email address for the purposes of facilitating your access to the Skin Check and sending you a feedback form and survey in relation to your experience. The information you provide may also be forwarded to the Clinic when you book online. For more information visit MoleMap’s Privacy Policy.
  • A de-identified aggregated report resulting from the Skin Check Vouchers used will be issued if 12 or more vouchers are redeemed.
